Is there a clean step by step tutorial how to deploy sitefinity with Load Balancing option on Windows Azure Role with latest Azure SDK?
Where I can find good explanation of why Sitefinity Load Balancing add-on is needed and what value it brings to the customer.

I have looked through Forums and Goggled for it. No clean explanation or tutorial.  Any help or direction is greatly appreciated.

Would be nice. Found your post while looking into the same for my implementation.

 How do I make sure all instances of my cloud service are added to Administration » Settings » Advanced Settings » System » LoadBalancing » WebServerUrls automatically.

 Sitefinity Load Balancing add-on would bring the ability to scale your website. Or in the case of Azure, you need to have 2 instances running on your Azure Cloud Service to get their 99.9% uptime guarantees. To me that's a must to meet MS's minimum requirement of customers using their platform to enjoy the benefits of using a cloud service.

Hi,

When the site is deployed on Azure, the load balancing is automatic and you do not need to perform any manual configuration. If you have Load Balancing in your license and the Cloud Services has more than one instance, the Load Balancing module will automatically send the messages to all nodes.
