1) You can use the article you have found and create your own class. Then use this class instead of the ASP.NET one in the create user wizard user control.
2) You can either expose properties to set this through the control editor, or set this inline in the template.
I have attached a sample control to illustrate what you should do. I have also exposed some properties for sample so that you can see for example how to set the destination page of the continue button through the control editor.
