Unable to load created forms

    When I am in the Page Editor for any given page, and I drag the "Form" content control onto the page and then click "Select a Form", I get the form dialog but it never stops "loading..." and a prompt comes up on the screen. See the attached screen shot.

    Dear Eric

    I guess it would help Telerik if you provide your OS, Browser and Version used. As well as a screenshot where you can see your forms.

    Hello Eric,

    Can you use Firebug plugin for Firefox and see what exceptions do you get in the Net tab. Also please share more details about your project - did you upgraded or it is brand new?  Also please tell me what have you done prior to receiving the error? Any additional information would be helpful.

    Hi Victor,

    This is a project upgraded from 3.7. Currently 4.0 and because of difficulty I've had upgrading to 4.1 and project schedules, I'm unable to do that at this time.

    Under Content > Forms, I've created three forms successfully. When I go to Pages and select a page I'd like to add a form to, in the Page Editor I drag the Form content control onto the page. Then I click "Select a Form" which is when I receive the prompt in the screenshot.

    Here is the exception from the Firebug Net panel:

    Hi Eric Zanol,

    Can you open a support ticket and send me your project ( codebase and database ) so I can inspect it locally? The error seems to be nested in your database records and I would like to try to replicate the issue on local environment and locate the problem.
