    Hello Paul,

    I have tested the template follwing that article: http://docs.sitefinity.com/create-and-edit-widget-templates and it seems work fine (screenshot).
    The only change is the skin name, as Sitefinity skin is obsolete, I change it to "Default".
    <telerik:RadTreeView runat="server" ID="categoriesCombo1" Skin="Default" />
    <telerik:RadComboBox runat="server" ID="categoriesCombo" Skin="Default" />

    If you need to use external widget template file you can check the following articles:
    http://docs.sitefinity.com/for-developers-customize-the-markup-of-a-specific-widget
    http://docs.sitefinity.com/use-external-widget-template-file

    In case you have more complex scenario and want to extend the widget functionality you can find more information here:
    http://www.sitefinity.com/blogs/stanislav-velikovs-blog/2012/01/23/how_to_add_additional_logic_into_sitefinity_built_in_widgets
